Transforming primary care with remote monitoring: Luscii's success in Leeds

 

Luscii is proud to share the evaluation of an innovative remote monitoring pilot that brought together key stakeholders across healthcare and local government. The pilot was funded by NHS England, and led by NHS West Yorkshire Integrated Care Board (ICB) and Leeds City Council. It was delivered by two Leeds Primary Care Networks (PCN): Burmantofts, Harehills, and Richmond Hill (BHR) and West Leeds PCN. The evaluation of the pilot was led and delivered by Health Innovation Yorkshire & Humber. This collaborative initiative explored Luscii's remote monitoring platform as a solution to improve care for people with multiple long-term conditions, while reducing pressures on primary care services.

 

Addressing key challenges in primary care

Primary care services in Leeds, like many areas across the UK, face increasing demands due to an ageing population and the growing prevalence of long-term conditions. Managing patients with conditions such as diabetes, chronic obstructive pulmonary disease (COPD), and heart failure requires ongoing, resource-intensive monitoring, placing immense strain on GP practices and clinical teams.

The pilot aimed to address these challenges by introducing Luscii's remote monitoring solution to:

  • Empower patients to gain insight into and self-manage their conditions effectively.
  • Reduce unnecessary face-to-face appointments to free up primary care clinician time.
  • Improve outcomes through earlier identification of deterioration, enabling earlier interventions and reducing avoidable hospital admissions.
 

The pilot: A collaborative effort with Leeds PCNs

The remote monitoring pilot ran from July 2023 to June 2024, spanning a year in total, with around 9 months dedicated to active testing. Patients were enrolled through BHR and West Leeds PCNs, where Luscii's platform was implemented to address the needs of individuals with long-term conditions.

Supporting those who didn't have the necessary digital skills or equipment was a key part of the project. Digital Health Hubs were on hand to provide extra support to those who needed it, such as to help with using the equipment and setting up the app. Leeds City Council's Libraries Service also provided access to Apple tablets to those who required one.

During this time, patients used the Luscii app to log vital signs such as blood pressure, oxygen levels, and weight, as well as completing signs and symptoms questionnaires personalised to their long-term conditions. These readings were automatically shared with clinical teams through bluetooth-enabled technology, who received real-time alerts only when intervention was necessary. This proactive approach enabled patients to manage their health more independently, while allowing clinical teams to focus there attention where it was most needed.

The pilot demonstrated how a relatively short testing period could deliver measurable improvements in patient care and healthcare efficiency. 

Key outcomes

The Leeds remote monitoring pilot delivered benefits to patients and clinical teams alike, demonstrating the transformational potential of Luscii's platform in primary care settings:

  • High engagement and data recording: Patients actively participated in the remote monitoring programme, regularly logging their health data and engaging with the Luscii platform, showing high levels of trust in the system.
  • Reduced demand for routine GP appointments: Within one PCN, the pilot was associated with a 54% decrease in routine GP appointments, suggesting the potential to free up clinician time for patients with more complex or acute needs.
  • Enhanced clinical insights and management: Patients saw improved blood pressure control and earlier detection of potential deteriorations, enabling more timely interventions and reducing the likelihood of condition escalation.
  • Potential for fewer emergency hospital admissions: The proactive approach facilitated by Luscii contributed to more efficient care delivery, helping to reduce avoidable emergency interventions.
  • High patient satisfaction: Based on multiple surveys conducted before, during, and after the pilot, patients consistently reported feeling more in control of their health. Many participants highlighted increased confidence in managing their conditions and reduced anxiety about their health.

This has not only helped enhance patient outcomes, but also helped to streamline workflows, making the jobs of clinicians more effective and rewarding. 

From health and council colleagues, to participating patients, the whole project has been a great demonstration of collaborative working within Leeds and hopefully opens the doors to greater use of tech within the healthcare system in Leeds."

We co-designed the pilot at every step and it's great to see the positive impact of the partnership one year on. Through this co-design, we managed to cater to a range of patients with complex health needs, who might have more than three long-term health conditions. We sought to support both patients who might have frequent GP appointments, as well as those who don't seek care, even when they're experiencing an exacerbation. The pilot has given us the chance to explore not only the benefits of remote monitoring, but also the process of implementation, which will no doubt make scaling up smoother and more sustainable."
